By filling out this form and submitting it in any way to the maritime agency “L-stream Ltd.”(LMS), the submitter thereby gives his consent, in accordance with the Law of Ukraine “On the Protection of Personal Data” dated 01.06.10 No. 2297-VI, to the  processing data (collection, registration, accumulation, indefinite storage, change, depersonalization, destruction, updating, adaptation, use, distribution, sale, transfer in full or in part to administrators or third parties) without additional written notification to your address, in order to provide the company with intermediary services for its employment on the ships of domestic or foreign shipowners, regardless of the country of their location.
